Read the notice or sign in to your official application to find the exact reason. Do not create several new applications. Correct any name, date of birth, Social Security number, address, eligibility, document, or household issue, then submit only the requested information through the official portal or your selected provider. Denied, Pending, Incomplete, Expired, or Already Enrolled?
These statuses require different actions. Starting a second application can create confusion when the first application only needs a document or is still being reviewed.

Seven Lifeline Application Issues and How to Fix Them
Name, Date of Birth, or SSN Does Not Match
The National Verifier may be unable to confirm identity when the information does not match official records.
Fix: Compare the application with official documents. Correct spelling, suffixes, hyphens, date format, and number entry. Submit an accepted unexpired document only when requested.
Program Participation or Income Was Not Verified
Automated records may not confirm SNAP, Medicaid, SSI, housing assistance, Veterans benefits, Tribal programs, or household income.
Fix: Submit the requested benefit or income document. Program proof should show the person’s name, program, issuing organization, and a recent issue date or future expiration date.
The Document Is Clear but Missing Required Details
A readable image can still fail when it omits a name, program, annual income, date, complete page, or issuing organization.
Fix: Follow the official acceptable-document guide. Upload a complete copy and do not alter required fields. Send copies, never originals.
Your Residential Address or Location Was Not Verified
Address matching can fail for rural, Tribal, shelter, or other nontraditional locations.
Fix: Use the mapping tool when prompted. Depending on the error, an accepted ID, utility bill, lease, tax return, government letter, or mapped location may help.
Another Benefit Is Associated With Your Address
Lifeline allows one benefit per economic household, not automatically one benefit per street address. Separate households can live at the same address.
Fix: Complete the Lifeline Household Worksheet when requested to show whether income and expenses are shared.
You Are Already Enrolled With Another Company
An active Lifeline benefit can prevent a second enrollment because only one company can apply the benefit at a time.
Fix: Ask the new company about a benefit transfer. Contact Lifeline Support if you did not authorize the existing enrollment.
The Application Is Incomplete, Pending, or Expired
An unfinished application may need documents. A pending application is under review. An eligibility decision may also expire before provider enrollment is completed.
Fix: Sign in and complete the existing application when possible. Follow official instructions if a current application is required.

What a Correct Supporting Document Should Show
Use the notice to identify what must be proven, then provide a document containing the required details.

Confirm That You Are Using a Current Lifeline Qualification Route
Age, disability, student status, or veteran status alone do not automatically qualify a household.

WIC and Pell Grant Clarification
WIC and a current Federal Pell Grant are not general qualification routes for every Lifeline applicant. They are among the extended routes for certain survivors of domestic violence, human trafficking, or related crimes under the Safe Connections Act, with additional requirements.
